SchoolInsight allows admins to populate Online Registration with pre-built forms that will gather necessary information for a student profile. There are some helpful state specific forms that can also be added for Illinois and Oklahoma.
The Standard Registration Forms are as follows:
- Student Information
- Parent/Guardians
- Emergency Contacts
- Home Language Survey (IL and OK Only)
Student Information
This form allows parents to enter student demographic information such as:
- Physical Address
- Phone List
- Middle Name
Unlike the subsequent forms, this form can be edited and fields can be added to gather more student information. The field Is this student going to be enrolled in this school for the upcoming academic year? is the only field that cannot be changed.
Parents/Guardians
The Parent/Guardians form is where information on primary contacts is added. This form has two editable fields for admins: a header that can be updated or removed and a setting, Allow contact information to be updated, which determines if primary contact information can only be viewed or if it can be changed.The information that can be updated if allowed is as follows:
- Name
- Relationship
- Address
- Phone Numbers
- Employer
- Can Pickup
Emergency Contacts
The Emergency Contacts form allows parents to add and update emergency contacts. This form has two editable fields for admins: a header that can be updated or removed and a setting, Allow contact information to be updated, which determines if emergency contact information can only be viewed or if it can be changed. The information that can be updated if allowed is as follows:
- Name
- Relationship
- Phone Numbers
- Employer
- Can Pickup
Home Language Survey (IL and OK Only)
This is a state required form for new students that gathers information regarding student Home Language in order to identify students whose families speak a language other than English at home. It also helps determine the students who need to be assessed for English Language Proficiency to determine eligibility for English Language Learner support. This form’s headers and field descriptions can be modified but not removed.
