How do I enter an examination?

This article explains how a nurse can enter an examination for a student. Examinations are given to students by outside healthcare providers and are entered into the Health Records Module. Within SchoolInsight, there are four types of examinations that can be entered:

Dental Examination

To Add a Dental Examination:

  1. Go to School Main > Health Records > Examinations
  2. Select Create Examination 
  3. Set an Examination Type
  4. Search for a student
  5. Enter an Exam Date
  6. (Optional) Select Requirements Satisfied by
    • Waiver Exemption: select relevant option 
    • Examination:
      • Select Form Version 
      • Services Provided: Select any services a student received during the examination
      • Oral Health Status: Select any applicable updates to the student’s oral health status
      • Treatment Needs: Select any future treatment needs the student may have as well as any appointment dates 
      • Comments: Write any additional comments given by the doctor
  7. Select Form Completed and Handed In
  8. Save

Eye Examination

To Add an Eye Examination:

  1. Go to School Main > Health Records > Examinations
  2. Select Create Examination 
  3. Set an Examination Type
  4. Search for a student
  5. Enter an Exam Date
  6. (Optional) Select Requirements Satisfied by
    • Waiver Exemption: select relevant option 
    • Examination:
      • Case History: Enter any provided ocular and/or medical history as well as allergies and additional information 
      • Examination: Enter provided examination details as well as any notes 
      • Diagnosis: Enter any diagnosis made as a Yes or enter in Other field if the applicable diagnosis is not provided
      • Recommendations: Enter any recommendations made as well as any comments, notes, and doctor information 
  7. Select Form Completed and Handed In
  8. Save

General or Sports Physical

The process to add a General or Sports Physical is the same. Below, we will list instructions to add a General Physical, but you can use the same steps for a Sports Physical as well.

To Add a General Physical:

  1. Go to School Main > Health Records > Examinations
  2. Select Create Examination 
  3. Set an Examination Type
  4. Search for a student
  5. Enter a Physical Date
  6. (Sports Physical only) Select Approved for Participation if applicable
  7. (Optional) Enter Notes
  8. Save
